- We are currently accepting newborns with private insurance and their siblings with the same coverage, after
we have reviewed the completed waitlist questionnaire.
- We are acccepting school teachers,
active military personnel (TriCare), and those privately insured and new to the community less than 6 months.
- All patients accepted into the practice
MUST
agree to our vaccine policy as stated
in the questionnaire. Failure do to so will result in immediate dismissal from the practice
- All patients, prior to final acceptance,
should contact the Appointment Desk to inquire about monthly "Consult Night."
Pregnant families are required to attend and should plan on
attending early, but we also know "little surprises" happen and we are
understanding of these special circumstances. Other families need to contact
the office to determine if attendance is needed. Please contact the office either by

- Privately insured families may complete the questionnaire and placed on a waiting list. We will be responsive to the wait list and you are welcome to check your
status.
- We are creating a newborn waitlist for Medicaid, Superior, Amerigroup and Chips and encourage you to submit the
waitlist questionnaire. We will respond
within 30 days as to your status.
- We are accepting others on a case-by-case basis, so again we advise submitting a thoroughly completed
waitlist questionnaire. We will keep you
informed.

NEW PATIENT QUESTIONNAIRE SUBMITTAL INSTRUCTIONS
Please read the section on Vaccines very carefully. You will be guided
through the process of electronic submittal but you also may print
a copy of a completed questionnaire or neatly complete by hand and
deliver to the office. We always prefer electronic submittal.

Go to the Questionnaire, after reading
instructions, using the link at the bottom of the page

After opening the questionnaire, fill in as many
blanks as possible and add any relevant information

After completing the questionnaire you should
select either "save as" or "print"

To provide an electronic version you must first
use "save as" which will be attached to an email generated at the website.

After selecting "save as" you will name the form
and save it.

Now choose "continue" on the form. This returns
you to the website for emailing
